Mental health care in India: private practice vs. public hospitals
India has one psychiatrist per 343,000 people. Here's an honest look at what mental health care access actually looks like, and how to find private therapists who are affordable.
India's mental health treatment gap — the proportion of people with a mental health condition who receive no treatment — is estimated at over 80%. This isn't a secret. The National Mental Health Survey of India (2016) documented it in detail. What's less discussed is why, and what you can practically do if you need care now.
The public system
Government hospitals provide psychiatric services, but waiting times are long and follow-up is limited. The quality varies enormously by state and institution. NIMHANS in Bangalore is a centre of excellence; district-level public hospitals are a different picture. The public system is better for acute psychiatric crises than for ongoing outpatient therapy.
Private practice landscape
India has a growing private mental health sector, concentrated in cities. Mumbai, Delhi, Bangalore, Hyderabad, and Chennai have the highest density of private psychologists and psychiatrists. Tier-2 cities like Pune, Ahmedabad, and Lucknow are growing. Rural areas remain severely underserved.
Fee ranges vary widely. A session with a psychologist in South Delhi might cost INR 3,000–6,000. The same session in Patna might cost INR 800–1,500. Online therapy from Indian psychologists typically costs INR 1,200–3,000 per session, making it one of the more accessible options nationally.
Things to check before booking
- RCI registration — The Rehabilitation Council of India registers clinical psychologists. An RCI-registered clinician has met government-mandated training standards.
- Specialisation — Ask whether the therapist has experience with your presenting issue (anxiety, grief, relationship difficulties, trauma) rather than assuming all therapists work on all problems.
- Language — India has 22 scheduled languages. A therapist who can work in Hindi or your regional language may serve you better than one who only works in English.
Online therapy platforms in India
